- IAPOA with dry seal and 3 specimen

5. Transcript of Records with Special Order “ For
Board Exam Purposes Only” with scanned photo
( original copy + Xerox with certified true copy )

6. NBI Clearance (PRC REQUIREMENTS )

7. Passport size colored picture with collar, white

background and with complete name tag ( Last
Name, First Name, Middle Name )

8. Php 900 fee + Php 30- metered documentary

stamp + Php 30- window envelope

9. College diploma ( original + Xerox with certified

true copy )
