Melissa Marr and Emily Maher of the State Engineer's ‘office inspected the HUFFAKER EFFLUENT STORAGE RESERVOIR. The purpose ofthe inspection was to determine the condition ofthe structure with respect to dam safety. The [HUFFAKER EFFLUENT STORAGE RESERVOIR is classified a8 a High hazard dam due to downstream development. ‘The overall condition ofthe dam is FAIR. Based on the enginee's observations, the following corrective actions should be taken: Short Term (1 Year) |, DOWNSTREAM FACE - Vegetation: Deep rooted vegetation and tees on the embankment face should be removed and the slash material deposited offite. - Photos Attached. Long Term (2 Years) 1, ALLDAM- Concrete Spalling/Cracking: During the inspection, concrete spalling/eracking was observed. Monitor these locations and repair as needed 2, INSTRUMENTATION - Piezometer/Observations Wells: This office recommends that the ‘owner start recording the piezometer readings at least twice a yea. 3.
