ACKNOWLEDGMENT OF PATERNITY

I, JIM C. BRAVO, 48 years old, married, Filipino citizen and

a resident of Purok Liloan , Brgy. Agusan Pequeño, Butuan City
under oath depose and say:
(1) That my wife, Sarah Bravo, gave birth to our
DAUGHTER, Jheniesah C. Bravo, in Butuan City on January 23,
1990, a time that me and Sarah were not yet married;
(2) That I was not around to acknowledge paternity and sign
HER birth records, as I was then working in Metro Manila;
(3) That on June 14, 1991, I and Sarah decided to tie the knot
in a Roman Catholic rite held at the Holy Redeemer Parish, Butuan
City;
(4) That until now my child, JHENIESAH C. BRAVO,
remain on record as my illegitimate and unacknowledged child
to legally use my surname since me and her mother, Sarah, is now
lawfully married;
(5) That I execute this affidavit to attest the foregoing fact
to ACKNOWLEDGE PATERNITY of Jheniesah C. Bravo, for the
purpose of her LEGITIMATION, and for such other legal purpose.
