I, Jomerto T. Flores, acknowledge that the Department of Electronics Engineering under the College of
Engineering and Architecture, accepted my request to be enrolled this First Semester of Academic Year
2018-2019, contrary to fulfillment of the retention policy.

Moreover, I will attend and participate on all assessments covered by the Professional Integration
subjects, however, will be only allowed to enroll the said subjects on the First Semester of the Academic
Year 2019-2020.
