RE: Henry O’keefe (82-year-old, DOB 02.02.1925)

Dear Director,

I am respectfully referring to your care Mr. Henry O’Keefe who is diagnosed with
Malignant Melanoma of left shoulder, he is in need of your full assessment
support at home. The patient was discharged from the centre last 17 March
2008.

Upon admission on the 14 March 2008, the patient had a large lesion on his left
shoulder which was successfully removed. On the 18 March 2008 during the first
home visit, his wound appeared to be healing well. However, he is showing
confusion with regards to his health status, he often forget his responsibilities,
unable to balance during ambulation and could not put things in order.
Subsequently, on the following day of visit his condition persist thus, his wife
Dorothy feel helpless and frustrated.

I would like to request that Mr. Henry’s home be visited by your team to assist
the couple with their basic needs. I shall do the dressing of his surgical wound
until his sutures is scheduled to be removed at OPD on 24 March 2008. I would
be grateful if you could arrange someone to assist him with showering and for
Dorothy that she be help in facilitating their house, garden , grocery and more
importantly in preparing their meals.
