AFFIDAVIT OF UNDERTAKING
I, ______________________________________________, of legal age, Filipino,
married/single, and a resident of ___________________________________________, after
having been duly sworn to in accordance with law, do hereby depose and state that:

1. I am a bona fide sugarcane farmer and a resident of the above-written address;

2. I intend to avail of the Socialized Credit Program of the Sugar Regulatory

Administration (SRA), which program is under the Sugarcane Industry
Development Act (SIDA) of 2015, coursed through the Land Bank of the Philippines
(LBP);

3. I undertake to comply with all the requirements under the Socialized Credit
Program, including the terms and conditions in the approved implementing rules
and regulations agreed upon by the Socialized Credit Program and the Land Bank of
the Philippines;

4. As applicant of the Socialized Credit Program, I further manifest and attest to

the fact that I have not previously availed of nor have I been a beneficiary of any
form of financial farm assistance or fund from any government institution, agency,
instrumentality or office, both local and national, and that, should I later on become
a beneficiary of the same, I undertake to immediately disclose such fact to the SRA
or to the LBP;

5. As beneficiary of the Program, the loan shall be used primarily on Sugar

Production or necessary thereto; that the transactions pertaining to such end and all
produce/proceeds of the harvest shall solely be under my name or my duly
authorized representative, when the need arises;

6. I am the exclusive owner and lawful possessor of the harvested sugarcane;

that I have the exclusive right to harvest mill and bring the same to the
________________ Sugar Central where I/__________________ of which I am
affiliated with, am/is under contract to mill;

7. I am executing this Affidavit to attest to the truth of the foregoing. This

instrument was thoroughly translated and explained to me in the dialect of which I
am conversant of, and that, I execute the same out of my own free will and volition.
