College Application Activity Log

Activity: Provide the name of the club, sport, job, or other activity. Write out the whole name (e.g.
National Honors Society instead of NHS). You may also be asked to choose a category provided by the
application (e.g. JV/Varsity sports, Work (paid), Music, etc.)
Description: Assume the person reading your application has no idea what your activities are. Briefly
describe what is involved in that activity.
When did you participate? Fill in the grade levels you participated and whether you participated during
the school year or over a school break.
Weeks per year and hours per week: This helps the application reader understand the time
commitment you had in this activity. Be honest about the average amount of time you spent on this
activity over the course of a year.
Positions held, awards, shared leadership: List any leadership positions you held and any awards or
honors you may have earned. Shared leadership means you were a co-captain, co-president, co-
manager, etc. This is also where you should list any district, state, regional, or national competitions, as
well as if you were the founder of a club or group.
You do not have to fill every space! Think about which activities are most important to you.
Activities to consider: School clubs, volunteer work, paid work, sports, family responsibilities (including
watching siblings or caring for a grandparent), music groups, theater, church responsibilities, and don’t
forget to include MSUB!!!
