This includes the supply of labor, materials and tools necessary to complete the laying of 600mm x 600mm Non Skid ceramic floor tiles at Executive Lounge, Pre-Departure Male Toilet and Public Toilet and 600mm x 600mm homogenous granite wall tiles. This item also include the replacement at Executive Lounge Area of the following: 3cunits water closet 3.umits lavatory 3uunits faucet 2-units urinal 3.units provision of granite tiles 10-un its soap dispencer 10-units toilet signages Installation of Phenolie board with complete accessories Replacement of 2-unit doors at toilet executive lounge area 10, Soil & Water pipe lines 11. Mirrors: and all other related items needed for the completion of this item. (Please refer to plans). Sean eeE Project Coverage: Toilet Floor Area 58,05 sqm. Toilet Wall. Arca 2 188.84 sq.m. CHIPPING AND DISPOSAL OF EXISTING CONCRETE and TILES. This includes supply of labor, equipment and tools necessary for the removal of existing deteriorated floor tiles and cleaning of excess tile grout. Chipping and disposal of the bIv. existing concrete to level the existing surface prior to the concrete topping and laying of tiles needed for the completion of the project. Project Coverage: Total Area: ‘1,791.00 sqm CLEANING AND DRAINING OF EXISTING SEPTIC VAULT. This includes the supply of labor, equipment and tools necessary to complete the cleaning of six (06) units of existing septic vault including declogging of existing soil and water pipes needed for the completion of the project. ‘TEMPORARY FACILITIES. The contractor shall supply the following provisions within ten (10) calendar days upon receipt of the Notice to Proceed (NTP), A. DOTC STAFF HOUSE. This includes the maintenance of the DOTC staff house including the provision of office supplies, office tables and supply of potable water. B. SERVICE VERILCE. This covers the provision of service vehicles at least 2005 model in good running condition and updated registration om a rental basis including driver and twelve (12) liters of fuel pet day for the exclusive use of the DOTC-PMO in supervising the project for the period of one hundred fifty (150) calendar days. Maintenance and fuel cost for the service vehicle shall be included under this item. ‘The contractor shall be responsible for all laboratory, materials testing’s necessary in the project implementation. Expenses for the said testing shall be incorporated in the contractor's overhead cost and shall not be considered as pay item. 6
