Iowa law requires any student entering kindergarten to provide the school with proof of a dental

check-up. There is a State approved form that all dental offices have on file. If you don’t return
the completed form the School Nurse will be providing a dental screening. If you do not wish
your child to receive this free service, please contact the school secretary by fill in date you
prefer.
