JOINT AFFIDAVIT OF EMPLOYMENT

We, VILMA MIJARES, LEONORA PREREZ BAYLE and ROBERT ANG,
Filipinos, of legal ages, married, and residents of Morga Hills,
Guinobatan Albay, P-3 Bonot, Legazpi City and P-2, Bonot,
Legazpi City, respectively, after being duly sworn in accordance
with law, depose and state that:

1. We are employed by Family Kitchenette with office address

at Brgy. 39, Bonot, Legazpi City, Purok 2, Albay on August 15,
2015 and January 19, 2016 for Vilma Mijares;

2. In connection thereof, we are registered to become bonafide

members of the Social Security System (SSS) with assigned SS
Number SSS No. 0504355620, 3374517653 and 0504340473;

3. In truth and in fact, we are only employed in the said

establishmen starting from August 15, 2015 and January 19, 2016
for Vilma Mijares up to present;

4. We are executing this affidavit to attest to the truth of

the foregoing and to inform the SSS of our true/correct duration
of employment with the Family Kichenette and for whatever legal
purposes it may serve.
