No real change as you will receive the same level of excellent care you have always re-
ceived from the doctors, who remain the same, and staff at Moana Medical Centre.
What happens to my medical records?
For patients who are routinely seen by the doctors at Moana Medical Centre, copies of
your medical records will be transferred to the new owners of Moana Medical Centre.

What if I don’t want my notes transferred?


If you do not wish to have your records transferred, please let your doctor or reception
staff know so that your records can be excluded from the transfer. If, at a later date,
you change your mind, a copy of your medical record will be made available to the new
owners once you have signed an authority release form.

What if I want to use both Moana Medical Centre and Aldinga Medical Centre services?
We always recommend using the same practice for your health care management as all
your records are available to the doctors who work there and they can provide you with
continuity of care. Seeing more than one practice may result in care that can be incon-
sistent. If you choose to see doctors at Aldinga Medical Centre as well as Moana
Medical Centre, we will, with your permission, send a copy of your consultation to your
usual doctor.

What about my recent test results?


Test results that arrive prior to the transfer will be part of your medical record. Any re-
sults that arrive after the business transfer will be made available to your doctor.

Willunga Environment Centre is joining millions of people
from 159 countries around the world in getting rid of throw- Est 1997
