untuk mengajukan pengunduran diri.

Niat saya ini berlandaskan pada kesadaran akan kesehatan saya sekaligus saya dan
keluarga harus pindah keluar kota disebabkan ada persoalan keluarga yang tidak dapat saya
ceritakan dalam surat ini.

Surat ini saya buat sesuai dengan prosedur hubungan industrial dan tanpa motivasi
negatif dari siapapun. Saya berharap perusahaan dapat memahami kondisi saya dan
memberikan hak–hak saya sesuai ketentuan yang telah berlaku.
