Clowns Fabrication Agreement

COSTUME DESIGN I understand that the costume design has been approved by Mrs. Stricklin,
Mrs. Atkins, and the design team. The design cannot be altered in any way without the approval
of Mrs. Stricklin, Mrs. Atkins or the design team.

COSTUME NOTES After the first costume parade, detailed notes will be taken on each Clown’s
costume to determine what, if any, changes need to be made. I understand that these changes
must be made according to the specifications presented by Ms. Fleischman and the design team
prior to the next costume parade.

DAMAGED OR LOST COSTUMES Normal wear and tear on costumes is expected. Damage
beyond normal wear and tear will require immediate attention. The costume must meet the
expectations of the design team and repairs or reconstructions must be made.

I have read and understand the Costume Fabrication Agreement above and agree to abide by
them. I agree to follow the costume design and not make any alterations to the design. I agree to
make any changes or additions to the costume that have been deemed necessary by the design
team. I understand that if costume is damaged beyond normal wear and tear that I am responsible
for making any repairs.
