would be a good idea to publish the list of
drug doses that | had compiled for my own
use. Alan wrote the resuscitation table and
suggested the inclusion of normal values.
The antibiotic guidelines were adapted from
notes written by Mike South. Stephanie
Brumby, Rob Henning, Mike Clifford, Trevor
Duke, Johnny-Millar, Jim Tibballs, Warwick

Abacavir (ABC). 8mg/kg (adult 300mg) 12H oral.
Abacavir 600mg + lamivudine 300mg. >16yr: 1 tab daily.
Abacavir 300mg + lamivudine 150mg + zidovudine 300mg.
>40kg: 1 tab 12H oral
Abarelix. Adult (NOT/kg): 1 vial (delivers 100mg) IM on days
1, 15 and 29, and then every 28 days.
Abatacept. NOT/kg: 500mg (40-60kg) 750mg (60-100 kg) 19
(>100kg) IV over 30min day 1, 2wk, 4wk, then every 4wk.
Abciximab. 0.25mg/kg IV stat 10min before angioplasty, then
0.2meg/kg/min (max 10meg/min) IV for 12hr.
Abiraterone. Adult (NOT/kg): 1g daily + prednisolone 5mg
12H oral. Monitor hepatic enzymes.
Acamprosate. Adulf (NOT/kg). <60kg: 666mg mane, 333mg
noon and nocte oral. >60kg: 666mg 8H.
Acarbose. 1-4mg/kg (adult 50-200mg) 8H oral,
Acebutolol. 4-8mg/kg (adult 200-400mg) 8-24H oral.
Aceclofenac. 2mg/kg (adult 100mg) 12H oral.
Acemetacin. 1.2mg/kg (adult 60mg) 8-12H oral.
Acenocoumarol. See nicoumalone.
Acetaminophen. See paracetamol.
Acetazolamide. 5-10mg/kg (adult 100-250mg) 6-8H (daily for
epilepsy) oral. TB hydrocephalus: 25-50mg/kg 6-8H plus
frusemide 0.25mg/kg 6H; may cause severe alkalosis.
Acetic acid. 1% 3 drops/ear 8H. Box jellyfish: apply vinegar.
Acetohydroxamic acid. S5mg/kg (adult 250mg) 6-8H oral.
Acetyl sulfisoxazole. 1.169 = 1.0g sulphafurazole (qv).
Acetylcholine chtoride. Adult (NOT/kg): 1% instil 0.5-2ml into
anterior chamber of the eye.
Acetyicysteine. Liver failure, paracetamol poisoning (regard-
less of delay): 150mg/kg in 5%D 1V over thr; then 10mg/kg
dhe for 20hr (delay <10hr), 32hr (delay 10-16hr), 72hr (delay
>16hr) and longer if still encephalopathic; oral 140mg/kg
stat, then 70mg/kg 4H for 72hr. Monitor serum K*. Give if
paracetamol >1000umol/L (150mg/L) at 4hr, >500umol/L
8hr, >250umol/L. 12hr. Lung disease: 20% soltn 0.1mi/kg
(adult Sml) 6-12H nebulised or intratracheal. Meconium ileus
equivalent: 5mi (NOT/kg) of 20% soltn 8H oral; 60-100mi of
50mg/ml for 45 min PR. CF: 4-8mg/kg 8H oral.
Acetyicysteine 5% + hypromellose 0.35%. 1 drop/eye 6-8H.
Acetyisalicylic acid. See aspirin.
Aciclovir. EBV, herpes enceph or sepsis, immunodef, vari-
cella IV over Thr: 20mg/kg 12H (<30wk), 8H (30-35wk);
236wk-12yr 500mg/m? 8H; >12yr 10mg/kg 8H. Cutaneous
herpes IV over thr: 250mg/m? (birth-12yr) Smag/kg (>12yr)
8H. Genital herpes (>12yr NOT/kg): 200mg oral xS/day for
