Dear Madam:

Greetings of peace and joy!

I, JEZALYN D. LIM, a grade II teacher at Calauag East Central Elementary
School - Calauag, Quezon, understand that there will be a Region-wide training
on Early Language Literacy and Numeracy on October 19-28, 2018 and based
on the memorandum I will be a part of the said training.

I am very thankful and blessed to be chosen as one of the teacher–

participants, but I apologize to inform your good office that I would not be able
to attend the 10-day training because of my health condition. I have undergone
a caesarian operation in 2017, roughly a year ago, and my lower abdomen is still
hurting especially during cold weather conditions. I have had frequent migraine
attacks as well and chilling during nighttime. In this regard, I am seeing my
Gynecologist during weekends.

With this condition at hand, I am afraid I would not be able to focus on

the training, especially it would be needing intense concentration. I have been
told that this is a relapse of my medical operation last year.

I am sending my intention to be replaced by whoever is willing and fit to

attend to the training.

I am hoping for your kind consideration to my request. Thank you very

much.
Respectfully yours,

JEZALYN D. LIM
