Natural Gas Service Commitment Letter

Dear Customer:

Thank you for starting the installation process for receiving natural gas service from Con Edison
Company of New York, Inc.

(A) Equipment Installation:

List the appliances/equipment you have contracted to convert to natural gas within six (6)
months of gas service being installed at the address below:

I agree in signing below to the following:
• I will complete installation/conversion of my equipment, listed in item (A) above, within
six (6) months from the date said gas service is installed by Con Edison Company of New
York, Inc. (“the Company”). Electronic notification of gas service installation will be
provided by the Company. Complete installation/conversion of equipment means that
the building is ready to have a Final Inspection conducted by the Company or its
contractor and pass the required inspection. I understand that if I do not complete my
gas conversion, I am responsible to Con Edison Company of New York, Inc. for the cost
of the installation of my gas service. This cost will be refunded in accordance with the
terms of the applicable tariff should I complete the installation/conversion at a later
date. Should force majeure prevent the completion of the Final Inspection within this
six (6) month time period, I agree to request, in writing, an extension from the Company
not to exceed sixty (60) days in length.
