Memo No. 11/102-4Dental-2013/568 -593 Dated: 15-03-2013 Subject: Installation of New Electrically Operated Dental Chaix/Unit with accessories in various health institutions of the State. Reference to the subject cited above. It is submitted that 160 new electrically operated Dental Chair/Units with accessories, are being procured by the department for various destinations in the State. The supply order was issued by Director Supplies & Disposal vide their memo No. 865/HR/G- 1/2012-13-54 dated 30-01-2013 to M/s Confident Dental Equipments Ltd. SCO No. 2444, Sector- 22, Chandigarh. ‘The present procurement is for electrically operated equipments. Since these Dental Chairs/Units are electrically operated, these are being installed at all the General Hospitals and Urban Dispensaries in the State so that they could be fully utilized. The Hydraulically operated Dental Chairs/Units, already installed in these institutions, are being shifted to the Primary Health Centres. In this regards, M/s Stardent, authorised dealer of Confident Dental Equipments Ltd. SCO No. 2444, Sector-22 C, Chandigarh, who is supplying the new equipments, has been assigned the job of shifting the hydraulically operated Dental Chairs to the Primary Health Centres and in turn install the newer ones at the destinations supplied by the department. The expenditure of shifting would be met out from Users charges of respective SKS of that institution in which the newer Dental Chairs/Units are being installed. Since the new equipment is to be installed at all General Hospitals and Urban Dispensaries so SKS of that hospital would bear the expenditure of shifting @ Rs. 3500/-+ taxes per Chair. [As per the Terms & Conditions supplied by the company, the hospital will provide the vehicle for transportation of old dental chairs/units to the concerned health institutions where these are to be installed, along with the Class-IV and Dental Assistant posted in the Dental COPD or otherwise. Senior Dental Surgeon/Senior most dental surgeon at district HQ, of the District Hospital will personally supervise the shifting of the equipments, so that the equipment may not damage. For intradistrict transfers Senior Dental Surgeon/Senior most dental surgeon at district HQ, of the District Hospital shall be fully responsible whereas for interdistrict transfers Senior Dental Surgeons/Senior most dental surgeons at district HQ, of both the District Hospitals will be responsible with their complete coordination, for hassle less installations. They will ascertain that the units are reinstalled in the same condition, in which they have been dismantled. In case of any query or controversy they must contact HQ. Concerned dental surgeon of the institution will get the dental chair/unit installed /reinstalled in his presence. Before dispatching, dental chair/unit, a proper stock entry in the central store be ascertained, later on they will be issued to the respective destination as per the list attached. A circular for providing water & electrically connection to all the health institutions has already been issued by this office on dated 22.02.2013, General Hospital SKS will issue a consolidated cheque to the company immediately after complete shifting of dental chairs @Rs. 3500+ taxes per chair, after taking a compliance report from concerned Senior Dental Surgeon/Senior most dental surgeon at district HQ, A list of the destinations from where thedental chair/unit are to be shifted, is annexed along with. Concerned Senior Dental Surgeon/Senior most dental surgeon at district HQ will be fully responsible for installation of new Dental Chairs/Units and reinstallation of old Dental Chairs/Units at the destinations mentioned. All the old Dental Chairs/Units would be reinstalled alongwith all the accessories. Term and conditions supplied by the company are enclosed herewith for information and necessary action. A copy of the destinations is attached herewith for necessary compliance.
