* All Parts Return request (received during the day) will only be processed by FSC
nightly between 1800hrs - 2300hrs.
* As such, at least 1 calendar day advance notice is required.
* DHL collection will be scheduled for next calendar day.

FSC to arrange DHL collection


(1) Service Request # / Case ID : (Specify Oracle SR number)

(2) Part Order Task ID = (Specify Part Order Task)

(3) Requested Part Collection date & time (local country time) (please put X
accordingly)
(a) [ ] NEXT CALENDAR DAY between 0900hrs - 1700hrs
(b) [ x ] Specific date & time = 14/05/2018 0900 – 1800hrs

(4) Site Contact Person = (Site contact person whom DHL will pickup the parts
from)

(5) Site Contact Number(s) = (Contact number of the On-Site Contact)

(6) Collection Address = (Address of Pickup)

(7) Parts Information (Note = please use separate line for each part number)
(a) Part Number = (Faulty part number), Quantity = (Number of Part being returned),
Part Status = (Specifi Faulty or Good)
(b) Description = (Faulty part description)
(c) Tracking No : (The AWBB number, the tracking number on the top of DHL form - on
top of the bar code)

(8) Any Special Instructions (eg CPAS ref number )
