Model Release Form

assigns, licensees and legal representatives the irrevocable right to use my name
(or any fictional name), picture, portrait, or photograph in all forms and media
and in all manners, including composite or distorted representations, for
advertising, trade, or for any other lawful pur-poses, and I waive any right to
inspect or approve the finished product, including written copy that may be created
in connection herewith. By signing this release I give Newco and their assigns,
licensees and legal representatives the irrevocable right to edit, modify, add to
and/or delete material, juxtapose any part of the film with any other film, change
the se-quence of events or of any questions posed and/or answers, fictionalize
persons or events including the releasee and to make any other changes in the film
that Newco deems appro-priate. In consideration of ___________, receipt of which
is acknowledged here. I have read this release and am fully familiar with it
contents.
