1. GUIDELINES FOR FILLING APPLICATION FORM 
 
1.1 General Instructions 
 
a)  Please refer to item wise instruction before filling up the form. 
b)  Fill in the form in English using CPAITAL letters, except for signature. 
c)   Use only BLACK or BLUE Hi‐Tec type pen to fill up the form. 
d)    Square  boxes  provided  in  the  forms  are  only  for  writing  the  alphabets  in  capitals  or  numbers.  
Only one alphabet or  number should be written in one box. Wherever codes are given, fill the  
boxes with the appropriate codes. 
e)  Overwriting,  striking  off  or  erasing  in  any  of  the  forms  may  lead  to  rejection  and  should  be 
avoided. Any discrepancy in the statement and/or submission of incomplete forms will lead to 
rejection of application/cancellation of admission. 
f)    Keep  a  photocopy  of  the  filled  application  for  future  reference.  Application  number  must  be  
quoted in all future correspondence. 
g)  Applicants who are required to submit photocopies of marks cards, certificates or any other  
documents must ensure that: 
 The photocopies are taken on A4 size paper only. 
 The print is clear and legible. 
 Both sides are photocopied if the original document is printed on both sides. 
 Application number must be written at top  right corner of all the photocopies. 
 
h)  Do not fold the form. Do not staple or clip the form with any other enclosures, keep it loose. All 
marks cards and  certificates if any must be stapled together. Demand draft must be kept loose. 
i)  The demand draft/at par chaque should be drawn in favour of 'Sikkim Manipal University' payable 
at Gangtok. 
j)  The application number and candidate's name should be mentioned on the reverse side of the 
demand draft. 
k)  Fees once remitted shall not be refunded under any circumstances.

Candidates seeking admissions through entrance examination of SMU can use one application form 
for either two group of courses or two mode of examinations (online/offline) for technical courses.

2. SUBMISSION OF APPLICATION FORM
i). No document to be attached with the application except DD as may be the case.
ii). You can send your filled application form either by speed post or by courier service.
iii). SMU is served by BLUE DART/ DTDC Courier service and Speed Post.
iv). Entrance Exam Fees
a). Rs. 1,000/- : For UGET1 or UGET2(online/offline)
b). Rs. 1,500/- : For UGET1 and UGET2 (Online) or UGET1 and UGET2 (offline) or
UGET2 Online and Offline.
c). Rs. 750/- : For BCA/MCA/MBA
d). Entrance Exam fees should be paod by DD / At Par cheque in favour of Sikkim
Manipal University payable at Gangtok.

3. Scratch Card
The scratch card carries an application number and password which enables respective candidate
to access the details/status of application/ online booking of centers/ hall ticket/ call letter from
the site www.smu.edu.in/apply. Also the same can be downloaded

i). By scratching this card, you agree to be fully responsible for all access, use and activities
that occur under your online booking password.
ii). The security of your online booking password is totally your responsibility. Sikkim
Manipal University (SMU) will not be held accountable or responsible for any claim or
loss that results directly or indirectly from any unauthorized use or misuse of your online
booking password.
iii). The online booking password must be kept confidential. Do not disclose with others,
write them down, store them in a file on your computer, or auto save passwords.
