(RFR) EVALUATION REPORT

B. Technical specifications of the antenna (See Annex A-1 and A-2)

C. RFR application fee of PHP 900.00 per transmitter site

D. For contractors, a notarized sworn undertaking (See Annex A-3)

E. Conceptual/Elevation Drawing

F. Floor plan for indoor antenna

Note:

The applicant facility shall submit a hard and soft copy (Word file) of the requirements listed
above.

A maximum of 10 sites per application and a maximum of 5 applications per contractor will be
accepted per day.
