Dear Dr. Combista:

The COTE Accreditation Task Forces have started gathering the necessary documents
for the AACUP scheduled visit which will be on July 6-10, 2015. However, these task forces
are also busy with their teaching load assignments, and therefore they were just using their
spare time to work on the accreditation documents. Since the schedule is too close we are
afraid that we could not make it on our target date.
In line with this, the undersigned would like to request approval from your good
office the granting of SERVICE CREDITS to the COTE Accreditation Task Forces during
Saturdays and Sundays in order for them to gather and package the documents. The
following are the proposed dates; June 20, 21, 27, 28 and July 4 & 5, 2015. The undersigned
assures close monitoring and supervision of these task forces during these days, so that
activities to be undertaken will focus for the intended purpose.
