AFFIDAVIT OF SUPPORT AND CONSENT

I, EVELYN SOLIS GENOBANA, Filipino, of legal age, and presently

residing at Block 13, Lot __, Silver Creek Subd., Upper Carmen, Cagayan de
Oro City, after having been sworn to in accordance with law do hereby depose
and state – That:

1. I am the mother of DAEVEN HEART S. GENOBANA, a minor 17 years

old, born on _____________________ and a resident of the same address.

2. I am giving full consent and authority to my son, DAEVEN HEART S.

GENOBANA to apply for a passport with the Department of Foreign
Affairs, Cagayan de Oro City.

3. I am giving my full consent to my son to travel abroad and he has my full

financial support during our travel abroad early next year accompanied
by me and my husband, as traveling companions.

4. I am willing and able to maintain and support my son financially and

guarantee that he will not become a public burden during his stay
abroad.

5. I am executing this affidavit to attest to the truth of the foregoing and for
whatever legal purpose it may serve.
