• First fill online form and on online submission you will receive a filled form print this form, print this
application form and sent along with required documents and paid voucher of Rs. 2500/- at any HBL
branch Account no. 0065-79007294-03
• This should be submitted to Admission cell, Liaquat University of Medical & Health Sciences,
Jamshoro, Sindh through courier service (TCS).
• Application forms will be available from 28-01-2019 on website www.lumhs.edu.pk
ELIGIBILITY CRITERIA AND MERIT FORMULA:
• Matric Science or Equivalent examination 10% weightage.
• Minimum marks of 60% in Inter Science Pre-Medical or Equivalent examination 40% weightage. (only on
Science subject Biology, Physic, Chemistry)
• Only those candidates are eligible who have appeared in Entry test conducted by NTS on 28th October, 2018,
50% weightage.
• The Domicile Student should be of the tagged districts of Sindh Province.
• Father Domicile may be of any district of Sindh Province.
• Those candidates who have applied earlier for admission in Public / Private Sector Medical colleges of Sindh
are only eligible.
• Put your NTS roll no. on the form
