INCIDENT REPORT

December 7, 2018

Dated December 8, 2018 at exactly 3:41 pm in the afternoon, Donnabella N. Ayco one of the
utility personnel reported to SG Nereus Alvarez that Stella Rose Gonzalez, a BSHM student
informed her that she speculated something in the comfort room. The said comfort room was
situated in building B. Thus, to verify the report, they immediately visit the area to confirm the
given information. There was found an estimated one (1) month old fetus inside the garbage can
of the cubicle. This was also verified and confirmed by Dr. Charlen Pol B. Anlap, the Campus
Administrator/School Nurse who was also around by the time the incident was reported. The fetus
was then transferred subject to be properly buried. The scene was then taken pictures for
documentary purposes to keep record of the incident and for whatever legal help it may serve.
