That I, DIVINA S. BARRERA, of legal age, Filipino citizen, and a resident of Blk. 2 Lot 9
& 10 Sagana Homes, Culiat, Quezon City, by these presents do hereby NAME, CONSTITUTE
and APPOINT MIGUEL FONTILLAR JR, likewise of legal age, with same residence address, to
be my true and lawful attorney-in–fact for me and in my name, place and stead, and for my
own use ad benefit, to do or perform all or any of the following acts and things, to wit:

1. To transact and follow-up my application for a renewal of my lost Professional


License card with the Professional Regulation Commission and claim the same upon
its scheduled release;

2. To receive, and acknowledge receipts; sign and execute necessary papers and
documents in relation thereto and generally to do or perform any act or deed
necessary or that may be required in furtherance of this authority;

authority to do and perform all and every act and thing requisite or necessary to carry into effect

REAL ESTATE MORTGAGE

KNOW ALL MEN BY THESE PRESENTS:

This Real Estate Mortgage, made by and between:

JUANA CHANGE, of legal age, single, and a resident of Solano


Hills, Sucat, Muntinlupa City (hereinafter known as the
MORTGAGOR);

- and -

ELLAINE GORIO, of legal age, single, and a resident of J. de


Leon St., La Huerta, Parañaque City (hereinafter known as the
MORTGAGEE);

WITNESSETH; That:

The MORTGAGOR is indebted unto the MORTGAGEE in the sum of One Million Two
Hundred Thousand Pesos (P1, 200, 000.00), Philippine Currency, receipt of which is
acknowledged by the MORTGAGOR upon the signing of this instrument, payable to the order of
the MORGAGEE without need of prior demand or notice within a period of six months, at Two
Hundred Thousand Pesos (P200, 000.00) monthly starting on February 15, 2016 and on every
15th of the month thereafter until fully paid, with interest thereon at the rate of 10% percent per
month.

NOW, THEREFORE, for and consideration of the afore-mentioned indebtedness, and


to assure the performance of said obligation to pay, the MORTGAGOR does hereby CONVEY
and DELIVER by way of MORTGAGE unto the MORTGAGEE, (his/her) heirs, successors and
assigns, the following parcel of land, together with all the improvements found and standing
thereon, more particularly described as follows:

A PARCEL OF LAND (Lot 20 Blk 54 of consolidation subdivision plan (LRC) Pcs-


13265, being a portion of the consolidation of Lots 4751-A and 4751-B (LRC) Psd-50533, Lot
3, Psd-100703, Lot 1, Psd-150980, LRC Rec. Nos. Nos. N-27024, 51768, 89632, N-11782, N-
13466, and 21071 situated in the Bo. of San Donisio, Mun of Paranaque, Prov of Rizal, Is. of
Luzon. Bounded on NE., point 4 to 1 by Road Lot 22, on...to the point of beginning; containing
an area of (280) square meters more or less..."

That it is the condition of this Mortgage that should the MORTGAGOR perform the
obligation to pay the afore-cited indebtedness of One Million Two Hundred Thousand Pesos
(P1, 200, 000.00) together with accrued interest thereon, within the agreed term, this Real Estate
Mortgage shall be discharged and shall at once become null and void and of no effect
whatsoever, otherwise, it shall subsist and remain in full force and effect and be subject to
foreclosure in the manner and form prescribed by law.

DEED OF DONATION

KNOW ALL MEN BY THESE PRESENTS:

THIS DEED OF DONATION, made and executed at Ilagan, Isabela this 19 th day
of March, 1977 by and between SPS. VICTORIANO TAN y VENTURA and ESTER
CLEMENTE-TAN, Filipino, resident of Calamagui 1st, Ilagan, Isabela, herein after
referred to as DONORS and TERESITA TAN-CASTEJON, of legal age, Filipino, and a
resident of Calamagui 1st, Ilagan, Isabela herein after referred to as DONEE.

WITNESSETH:

WHEREAS, the DONOR, VICTORIANO TAN y VENTURA is the registered


owner of a parcel of land situated at Calamagui, Ilagan, Isabela covered under Transfer
Certificate of Title No. T-54588 of the Registry of Deeds of Isabela, while ESTER
CLEMENTE – TAN, is the wife of Victoriano Tan y Ventura, more particularly described
as follows:
“A PARCEL OF LAND (Lot 3-B-1 of the subdivision plan LRC Psd-
140017, being a portion of Lot 3-B, LRC Psd-80380, LRC Rec. No. H.
Pat.), situated in the Barrio of Calamagui, municipality of Ilagan, Province
of Isabela. Bounded on the SW., NW., & NE., points 1-4 by Lot 3-B-2, of
the subdivision plan; on the SE., points 4-1 by Road. Containing an area
of THREE HUNDRED SEVENTY FIVE (375) SQUARE METERS, more or
less”

WHEREAS, the DONEE, being the eldest among the children of the said donors
hereby donates the above-described parcel of land including its improvements.

That the Donors do hereby states, for the purpose of giving full effect to this
Donation, that he has reserved for themselves in full ownership sufficient property to
support the family in their needs including their future inheritance;

That the Donee does hereby accept this donation of the above-described
property, and does hereby express gratitude for the kindness and liberality of the
DONORS.

CANCELLATION OF MORTGAGE

WHEREAS, by a certain instrument in writing, made and executed at Davao City on


March 25, 2003 by EMILIANA VDA. DE GARCIA (mortgagor), mortgaged to RICARDO V.
GARCIA (mortgagee), of legal age, Filipino, married to Maria Ingrid Garcia, and a resident of
Davao City, the real property covered by P-13154 for the sum of Two Hundred Fifty Thousand
(P250, 000.00) Pesos;

WHEREAS, the mortgagor certify that the above mortgage and the debt secured by such
mortgage are fully paid, and that the mortgage is fully and forever released from the property
described therein;

WHEREAS, the mortgagor release to the person or persons legally entitled thereto all
right, title and interest that the undersigned may have acquired in and to the property by reason
of such mortgage;
