2nd Scenario

A 68-year-old woman came to the outpatient clinic of a Public health center accompanied by her
neighbor. Patient looked thin and frail and require assistance when walking and sitting. She
complains of weakness, loss of appetite, depressed mood, decreased concentration and memory.
Occasionally she experiences panic attacks, in which she feels extremely anxious and fearful of
dying. She associates the onset of her depression with the death of her son 5 years ago. Two months
ago, her husband passed away, and she increasingly felt sad and depressed, and most of her time is
spent in bed. When examined by a nurse using the Geriatric Depression Scale, the score was 12,
and MNA 8. The nurse plans to refer the client to get antidepressants medication and Cognitive
Behavioural Therapy.
