CASE 1: Petrobras P-36 Accident

First Analysis: Assume that you were part of the organization, what
steps would you have done to prevent the incident to happen?
• As part of the organization, I will collaborate to the team, especially the
operations and maintenance department. I will implement a regular
checking of the indicators to be aware of time to time situations. I will also
provide a back-up plan for the unavoidable and unforeseen conditions,
such as response procedures for potential emergencies.

Second Analysis: Was the action taken sufficient to address the


impacts of the incident brought upon to the organization and to the
society as a whole? What recommendations will you give to address
the incident considering your profession as an Industrial Engineer?
• Petrobras P-36 accident is a case wherein the organization was lack of
planning procedures. First is the design structure of the offshore oil
production that cause malfunction of the operations. Next is the poor
maintenance system which leads to the explosion of Petrobras. As an
Industrial Engineer, we should always think of 80% planning and 20%
action. I will check the materials used if they are sufficient to handle the
pressure underwater because quality first before quantity. I will design an
emergency procedures which initially may prevent the explosion that can
claim crew member’s life. The incident also happened because of lack of
communication between the workers and in this point, we should train
people well making them understand the components and equipment
inside the production while empowering them that safety is very important.

CASE 2: The Failure of the Atlantic Telegraph Cable

First Analysis: Assume that you were part of the organization, what steps
would you have done to prevent the incident to happen?

Second Analysis: Was the action taken sufficient to address the impacts of
the incident brought upon to the organization and to the society as a
whole? What recommendations will you give to address the incident
considering your profession as an Industrial Engineer?
