Development Corporation, duly authorized to represent the corporation in
the filing of the instant petition pursuant to Board Resolution by the
Corporation as shown by the Secretary’s Certificate, hereto attached as
Annex “A”.

2. I caused the above Application to be prepared. That I have read,

and understood all the contents thereof;

3. All the allegations contained therein are true and correct to the
best of my own knowledge and authentic records;

4. I have not heretofore commenced any other action or

proceeding involving the same issue with the Supreme Court, the Court of
Appeals or different Divisions thereof, or any other court, tribunal or agency;

5. And if I learn of such similar action(s) or proceeding(s), I hereby
