Section of the Embassy of Brazil in Seoul, in relation to my visa
request, with powers to submit the application, pick up the final
documents, and give/receive related information.

I declare that, throughout the process, I will stay in the territory of the
Republic of Korea. In case the Consular Authority requests, I agree to
be interviewed, in person, at the Consular Section, and to provide
additional documents.

I acknowledge that, if my visa is issued, I must report to the Federal

Police Department of Brazil within 30 days of my first entry into Brazil.

The statements above also apply to the visa requests of my

dependents: (wife’s name, passport number) and (children’s name,
passport number).”
