AFFIDAVIT

I , ZENAIDA PACION GABUYO of legal age, married ,Filipino, and a resident of

Caggay Tuguegarao City, Philippines, after having been sworn, hereby depose and state:

1. That I am the daughter of “DANILO LUCAS PACION”;

2. That however, in his birth certificate, his first name was erroneously placed
therein as “DANILA” instead of his true and correct name of “DANILO” as
appearing in all his documents such as his marriage certificate;

3. That the error may have been caused by the inadvertence of the person who
prepared his birth certificate;

4. That the names “DANILO LUCAS PACION” AND “DANILA LUCAS PACION” as
erroneously appearing in his birth certificate, REFERS TO ONE AND THE SAME
PERSON;

5. That we execute this affidavit to attest to the truthfulness of the foregoing

statements and for whatever legal purpose or purpose this may serve under the
law.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this February 19,
