Loan Agreement

This Loan Agreement, is made as of this ____________(effective date) by Mellie Joy B. Jimenez and
between ___________(barrower), located at __________________________ (address).

The parties agree as follows:

Loan Amount
Lender agrees to loan the Barrower the principal amount of ______________, together with
interest on the outstanding principal amount of the loan and in accordance with the terms set
forth below. Maximum one-time loanable amount is P5,000.00 only.

Repayment of Loan
The loan together with the accrued, unpaid interest and other charges in case of unsettlement, is
due and payable on or before _______________.

Interest
The principal balance shall bear an interest at the rate of 6% monthly at a simple interest.

Late Fee
In case the barrower fails to make a payment due under this agreement within 7 days of grace
period after the due date, Barrower agrees to pay the Lender a late payment fee of P150.00 only.

Security
Barrower agrees that until the loanable amount with interest is unpaid in full, no immediate
additional loan is to be made.

Guaranty
The barrower declares _______________(name of co-maker/guarantor) at _________________
(address), swear to unconditionally guarantee the Lender the full payment of the remaining
balance of the principal payable amount and accrued interests.
