RELEASE AND QUITCLAIM

I, HENRY SIA, of legal age, married/single, with address at 10

Guava Road, Potrero, Malabon City, after having been sworn to in
accordance with law, hereby depose and state that:

1.) I was employed by Mr. HENRY SIA since 1990 up to my

retirement with the SSS upon reaching the age of 60/65
years;

2.) During my entire work with Henry Sia, I would be

regularly given monthly allowances and bonuses aside
from my regular salary;

3.) Upon my retirement, Henry Sia voluntarily gave me the

amount of ONE HUNDRED PESOS (P100.00) as gratuity
pay for my services rendered to him;

4.) For and in consideration of the said amount, I have no

more claim whatsoever from HENRY SIA arising from
my employment with him such as but not limited to,
unpaid wages, labor standard benefits, 13th month pay,
retirement benefits, separation pay and such other
benefits as may be due me under the law;

5.) I am executing this Affidavit of Release and Quitclaim

willingly and voluntarily and with full knowledge of its
consequences.

IN TRUTH WHEREOF, I have executed and signed this Release
