He does not come forward and tell the truth. He suffers immense guilt for his sin and his cowardice in
letting Hester and Pearl bear their ostracizing alone.

Because he keeps his sin hidden inside, he has health problems and at one point sees temptations
everywhere. He slowly grows more and more ill. Once he confesses his sin publicly, he dies, the
stress having killed him.

So we can say Dimmesdale was more racked by his guilt.
