Marriage Certificate If applicable. For women only
No need to translate. Simple copy
Valid Passport (at least 2 years validity) with notary

2) University Credentials
Diploma (Nursing)
Transcript of Records
Related Learning Experience
Course Description with total hours
(Nursing subjects only)

3) Professional Credentials
Active PRC Nursing License
Board Nursing Certificate
Certificate of Board Passing
Certificate of Good Standing
Certificate of Board Rating
Certificate of Employment with Job
Description (Hospital)

Note:
- Please photocopy all your documents in A4 size and give it to the translator.
- Translator must have German License. List of translators attached in the email.
- Notary can be done only by Atty. Falcon and German Embassy.
- All translated and notarized documents to be submitted in GIZ Triplewin office.
