School Dison Seperintendent For the Henge Superintendent RAFAEL IWIN G, NICOLAS ‘Administratfle Officer V SUBJECT: IMPLEMENTATION OF GRADE 12 CAREER GUIDANCE PROGRAM FOR SCHOODATAR 2019-2019, DATE: November 14, 2018 1, DepEd Memorandum No. 169, s.2018 dated October 31, 2018 issues the Career Guidance Modules on the Implementation of Grade 12 Career Guidance Program (CGP) for School Year 2018-2019, 2. The modules were rofled-out nationwide during the National Training of Trainers and Mass Training of Teachers and Career Advocates. The Grade 12 CGP Modules shall be delivered by the Grade 12 class adviser with the assistance of the school guidance counselors during the weekly homeroom guidance between September 2018 to March 2018. The school heads/prineipals, through the Youth Formation Coordinators in close coordination with the school guidance counselors/coordinators, / advocates, shall ensure and supervise the implementation of career guidance modules. 3. The schools are advised to deliver four modules each semester (First Semester: Modules 1 to 4; Second Semester: Modules 5 to 8), The Grade 12 Career Guidance Modules contain the following eight titles Entering the Exits; Discovering the Cs; The Choice of Choosing; Myself in another Person” shoes; Future Combo, Activated; Keep Me Balanced; Version of Me 2.0; and Ready to Take Off!The modules can be accessed through the DepEd Leaming Resource Portal. The school guidance counselor/designate shall accomplish the Monitoring Report Form provided in the said memorandum. The report shall be submitted to the Youth Formation Unit of the School Governance and Operations Division (SGOD) for consolidation on Mareh 22, 2019 for the eight modules.
