    2. Original Training Record Book (TRB)
    3. Certification of Six Months Bridge Watch keeping
    Duties Signed by the Master Mariner
    4. Certified True Copy of Crew List with Coast
    Guard Stamp (3 copies)
    5. Photocopy of Seaman's Book
    6. Daily Journal
    7. Photocopy of Passport with Immigration Stamp
    (Apprentice on International Vessel)
