AFFIDAVIT OF UNDERTAKING

I, EMILIA B. LOPEZ, Filipino, of legal age, a resident of Unit 511 Bandung Raya Condominium,
Paranaue, with official business address at Rm. 101 G/F Zen Bldg., Mayapis St., San Antonio
Village, Makati, after having been subscribed to an oath, depose and say:

I. That I am the incumbent President of Hiro Global Manpower, Incorporated, a domestic

corporation organized under the Philippine Laws, and a duly licensed recruitment agency as per
License No. LB-030217-R issued by the Philippine Overseas Employment Administration with
license valid until June 21, 2019.

II. That pursuant to Memorandum Circular # 04 Series of 2018, the recruitment agency must be
compliant with all the requirements in order to receive an Issuance Certificate to process
Technical Internship Training Program for Japan. The deadline of submission of the
requirements is on May 15, 2018.

III. That upon submission of the Board of Directors NBI Clearance at the Anti-Illegal Recruitment
Board of POEA last May 11, 2018 found a derogatory record of in the RTC of Camarines Norte,
the said record needs to be cleared in the province and was requested to submit a Police
Clearance from the Municipal Police Station which is now still in process.

That I, EMILIA B. LOPEZ, assure and take full responsibility on the submission of the said lacking
document for the filing of Issuance of Certificate to process Technical Internship Training Program
for Japan within Five (5) working days after the aforementioned date this Affidavit of Undertaking
was submitted.
