Greetings of peace in Christ Jesus!

We are the teachers of the Open High School Program of Agusan National high School
for the school year 2016-2017. We have been implementing this School Initiated
Intervention for three years . We have achieved positive responses from our clients and
community. We have also reduced our Dropout Rate for school year 2016-2017 that
can be seen in the LIS Data. And eventually the school has produced fifteen Junior High
School Completers who came from the Open High School Department during our 2 nd
Moving Up Ceremony last April of this year.

We are writing you this letter to request for service credits for the eight Saturdays that
we have rendered our service and reported to the School for our classes in the Open
High School Program.

We would be very much glad if you will grant our request. We are always inspired and
determined to serve the students in the Open High School Program.
