I am demanding that the information in my Chexsystems profile be validated. It contains several

unverified lines of information that have severely damaged my ability to obtain a proper checking
account. Due to your poor reporting, I have had to maintain a prepaid card which has resulted in me
incurring high fees.

You have 30 days to verify all the information in my report and to provide me with a document bearing
my original signature regarding any past due debt you say I have agreed to be held liable for. If you
cannot provide me with this information, I am demanding removal under the FCRA. If you choose not to
remove the information, please provide me with an address I can provide to my attorney to begin legal
proceedings to clear my name under my rights given to me under the Fair Credit Reporting Act.
