I agree to inform the Membership Team of any changes in circumstances during the subscription year
which may affect my eligibility for a reduced rate. My subscription will then revert to the full amount and
the balance will become due.

I understand that failure to do so may result in my referral for consideration under the IFoA’s Disciplinary
and Capacity for Membership Schemes.

 Applications from existing members must be made by 31st December 2018.
 Applications can take up to four weeks to process, therefore please ensure that you allow
sufficient time for processing.
 Surcharges will be applied in line with the subscription policy.
 You should not pay the full rate subscription if you intend to apply for a reduced rate.
