Notification No:- ()3 - SPMC of 2019. Dated: - BO - 03-2019. This is forthe information of all the Principals of AMT / ANMT/ GNM Schools, (Government and Private) that the examination of Para-Medical and Nursing Trainees of Jammu and Kashmir Divisions including Leh &Kargil shal be conducted in the month May’ 2019" for the examination session April May'2019 (Annual for Kashmir Division and Bi_Annual for Jammu Division including Leh & Karail respectively) as such they are accordingly enjoined upon fo submit examination forms ofall the eligible candidates with the following instrucions- 1 That the Principals concemed of AMT / ANMT Schools (Government / Private) should submit the examination forms of all the Candidate(s) registeterd during the previous session 2017- 2018, and ensure that they are registered with the J&K State Paremedical and Nursing Council or anyother eligible / Backlog candidate(6) ofthe examination Oct Nov '2018 by or before 30 — 04 - 2019, along with examination fee @Rs . 1000 Per Candidate in the shape of Bank Slip, thereafter no forms wil be accepted whalsoever the reason may be. ‘That the Principals of AMT / ANMT Schools and the Private Para-Medical Institutes concemed should verify thatthe examination forms of eligible candidates should be sent along withthe prescribed fee fo the council as per rules. That the Principals of concemed AMT / ANMT Schools Goverment / Private concerned while submiting the examination forms will examine and certify the eligibilly of the candidates in respect of age , qualification , Registration , Date of selection through nomination etc. ofthe candidate required as ber norms before submitting the examination form. intemal assessment of Marks should be sent with the examination Forms along with other particulars such as Name, Parentage, Address, of each student in a Pen-drive only and one Hard Copy of the examiniees(Stream wise) duly signed by the concerned Principal / Head of the Institution with official stamp. That the concerned head of the institution should submit the certificate or an affidavit in favour of the Examines that they have completed / undergoing the clinical training for the Particular class which He / She is going to appear(Certificate Mandatory to be submitted). by MA p\o Lo 6. That the Head of the concerned Institution will enclose a certificate to the effect that the examinee has got not less than 80% of attendance(Both Theory and Practical’). 7. No document should be attached with the examination form (except 1* Year Qualifying certificate) with two attested Photographs (for 2Year Candidates appearing for the examination). Howvever the candidate must fil ll the columns properly after evaluating the paticulars with their records. 8, That the examination of General Nursing & Midwifery 1s Year (Freshers) and 2%! Year (Lateral Entry) 2018 batch shall be conducted as per the newly prescribed syllabus by INC (Indian Nursing Council) and for other candidates the syallbus remains unchanged. 9. That the candidate must have completed at least one year in the said training course. 40. Vice President J&K State Para-Medical & Nursing Council (Principal / Dean GMC Jammu) for kind information with the request fo have the above notification circulated among all the concerned in Jammu Division with the instructions to ensure that the above directions are adhered strictly by all the Para-Medical Institutes of Jammu Province also. 3.
