BIENNIAL EXTENSION OF EMPLOYEE INTERCHANGE AGREEMENT BY AND BETWEEN THE UNIVERSITY OF SOUTH FLORIDA AND THE FLORIDA DEPARTMENT OF HEALTH This Biennial Extension of an Employee Interchange Agreement (the “Extension” is entered into as of the 1" day of September, 2017 by and between the University of South Florida Board of Trustees, a public body corporate ("University"), and the Florida Department of Health (‘DOH’). WHEREAS, Chapter 112, Part Il, Florida Statutes, authorizes the interchange of ‘employees between governments; WHEREAS, University and DOH entered into that certain Employee Interchange ‘Agreement dated September 24, 2013 (the “Agreement’) allowing the assignment of Douglas A. Holt, M.D., a University employee, to DOH to provide senior management services; WHEREAS, University and DOH signed an Extension in September 2015, extending the Agreement for two years through September 23, 2017; and WHEREAS, University and DOH desire to extend the Agreement an additional two years from its current expiration date. NOW, THEREFORE, for the mutual covenants contained herein and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties agree to extend and amend the Agreement as follows: 1. Notwithstanding anything in the Agreement to the contrary, effective September 23, 2017, this Extension authorizes Douglas A. Holt, M.D. to be assigned to the DOH to serve as the Director of the Florida Department of Health in Hillsborough County. 2. The Agreement is extended for a period of two years, to September 23, 2019. 3. Attachments 1, 2 and 3 to the Agreement are deleted in their entirety and new Attachments 1, 2 and 3 to the Agreement are inserted in lieu thereof as attached hereto.
