I would like to apply for a Tourist Visa to cover my travel to the Kingdom of Thailand from
11th August, 2017 to 16th August, 2017. I will be travelling in a group with the following

(v) Leave letter from employer
(vi) Proof of income (certified copy of bank statement for the last six months)
(vii) Travel insurance policy covering the entire period of stay
