> Certificate of Tax Exemption from the Bureau of Internal Revenue (BIR)
> Certificate of Indigency with Gross Annual Income of parent/guardian issued by the
Barangay Chairman
> Affidavit of No Income or Case Study from DSWD
> For children of Overseas Filipino Workers (OFW) and Seafarers, a latest copy of
contract or proof of income may be considered
B. For incoming Freshmen:
> For Graduating Senior High School applicants – (a) 1st Semester Senior HS Grades
AND (b) Certificate of Good Moral Character
> For Senior High School Graduates – (a) Form 137 AND (b) Certificate of Good Moral
Character
C. For Applicants with Earned Units in College - (a) Certificate of grades in all subjects in
completed semesters AND (b) Certificate of Good Moral Character
D. Other Applicants
>For ALS Passers – Accreditation and Equivalency Test Passer Certificate
>For PEPT Passers – Certificate of Advancing to the Next Level
E. For Applicants with single-parent, also submit Solo-Parent ID or Certification from
Department of Social Welfare and Development, if applicable
F. For applicants belonging to an Ethnic Group, also submit Certificate from National
Commission on Indigenous Peoples (NCIP)
G. For PWD applicants, also submit Medical Certificate
H. For applicants living with a guardian, also submit Affidavit of Guardianship
