Letter of Recommendation Form

Please give the letter writer 2-3 weeks to respond.

Please take the time to fill out all sections thoughtfully and completely. The letter writer needs to be able
to know enough about you so that they can write a recommendation that will make you stand out from
every other student applying to for the same scholarship or college as you.

1. What are your strengths (Please provide an example for each one)?
● I would say one of my strengths are not being afraid to push myself to do more and doing
whatever it takes to achieve where I want to be, for example my freshman and sophomore year
I would stay after school for tutoring to work on subject that I needed help on to improve my
grade in the classes I struggled in and having a good work ethic.
● Having a good relationship with my teachers I would say is a strength because without that I
don’t think i would have been able to ask questions in class when I would get confused on a
subject that we’re discussing in class and just always being able to ask questions.

2. Name one weakness that you have and what you are doing to work on it.
● Being a shy and quiet person has always been my one of my weakness things I’ve done to help, i
would say being a bit more engaged with the class and not so much as being quiet when being
asked a question or if I’m the one asking the questions. I’ve also started track so I think that his
helping because I’m getting feedback from my coaches and also some of my teammates as well.

3. Describe some of the activities (extracurricular, hobbies, sports, church, etc.) you’ve been
involved in over the past three years, and why they’ve been important to you.
● My freshman year I was part of student council and black student union (BSU), I wasn’t involved
as much in BSU due to student council that year but over the next few years I got more involved
with BSU by helping with volunteering activities, planning fundraisers for the club and getting
more of my peers to attend the meetings. As for hobbies I love playing chess I just think its a
great way to work your brain especially if you’re playing against someone better than you.
Making beats and even writing lyrics to the beats I would make on Garageband I like to do to
pass the time, I don’t do it as much as I used to but last year for my internship I had a change to
work on a rap album and make a music video for one of the songs in the album with spyhop, I’m
grateful to have that opportunity and I thank my internship teacher Martha for helping me with
that. I also enjoy editing pictures on app called picsart that I use, I also collect older comic books,
I’m a pretty big sneakerhead, I enjoy hanging out with family and friends of course and I join the
cottonwood track team this year to get my last p.e credit and to do a sport because I’ve always
been a athletic person and I regret not doing any sports my previous years but being the shy
person I am held me back from doing that.

4. What are your personal and academic goals?


● My personal and academic goals would be to graduate this year with my class, move forward to
get my generals done at slcc and then transfer to the U or Westminster and just be the best
version of myself I can be.

5. What do you plan to study in college, and why?


● I plan on studying pre-med so I can become a burn surgeon because I have a burn on my left
middle finger and when I had my surgery back in 8th grade during the summer, the process and
the way the surgeons explained what they we’re going to do fascinated me. I also think being
able to relate to the patient in a similar way would be good for them especially if it’s a younger
patient that has never had surgery before that is scared would help sooth them so their not as
scared and everything is better when you can relate to someone that has been in your position.

6. What do you see yourself doing 10 years from now?


● Hopefully I’m alive because anything can happen from now till then and for this scenario I am so
hopefully I’m either getting ready to graduate or I have already graduated and I’m working as a
burn surgeon.

7. If you asked your friends and family to describe you in just three words, what would they be?
● Funny, shy and smart

8. Describe yourself to your future college roommate.


● “Hey I’m Malachi I’m a pretty fun person and when serious things need to get done I will put in
the work to get it done so I think we’ll get along pretty well.”

9. Are there any obstacles that you have had to overcome (i.e. parent divorce, death of a loved
one, refugee, ESL, disability, first to graduate from high school, low income, etc…) and how have you
overcome the difficulty?
● One thing that I had to overcome would be being seen as “the troublemaker” all throughout
elementary and jr. high and being able have a fresh new start at ames helped me with that
because not as many people knew me so there wasn’t that thought of me of “oh, yeah he’s
gonna be the troublemaker” and just already having that thought about me that wasn’t true. I
would also say the obstacle of whether or not I needed to take medicine for my ADHD to help
me stay focus because I was totally against it and I thought it wasn’t going to help and I wasn’t
going to be myself if I took it but after fighting and being against it I finally gave it a try and I
would say it has helped me keep focus.
10. What community service/internship projects have you completed or organized? How many hours
were involved in each?
● I did a 7 month internship with spyhop, as for service projects they we’re all with BSU and the
ones I can remember was a clothing drive that we did last year I think for the youth center I
think we collected clothes for a few weeks and then we delivered them, this year we went to
the YWCA for about 2 hours and we just played with kids that were dropped off and those are
just the ones I can remember off the top of my head.

11. How would it feel, or what would it mean to you, to get this scholarship?
● If I was to get this scholarship it would mean a lot because paying for college isn’t the easiest
thing for me or my family so if I was to get this scholarship, it would make a big difference in
helping pay for college it would just also mean a lot to me and my family knowing that not all
the college expenses will be coming out of our pockets.

● I think I have put everything that I needed to put and everything that would help, if you have
