LIS (ALS)
Attention to ALS Facilitator with Unfinalized CY2017 CLCs:
1. Kindly update Individual Learner Status for CY2017 until May 31, 2018.
2. If the Facilitator is No Longer Available or in the position to update Learner Status, the ALS
System Admin can assign a replacement facilitator account to update learner status and
finalize CLC. Please refer to instruction stipulated in Deped Order 44. s.2017 section IV par.

3. 100% learners updated status and CLC finalized for CY2017 are pre-requisite procedure for
encoding CY2018 enrolment

ALS BOCY CY 2018 encoding is now open.

Instruction: All learners attending learning sessions from January 2018 shall be encoded in the
system.
Reminder for the Facilitator:

1. Facilitator must have finalized his/her CLS for 2017 to be able to access the CY2018.
2. Learner with no Status (complete/not complete) must update first to be able to enroll

Reminder for ALS system Admin:

1. Update the list of Facilitators and place of assignment.

2. Provide technical assistant to facilitators thru managing their accounts that includes but not
limited to resetting password
3. Coordinate with the division system admin for user access management system (UAMS)
concerns.

EBEIS
Updating of numbers of teachers and Science is now open.
Please be advised that this facility is only available for Public JHS and Public SHS schools
and until April 30, 2018.

LIS (K-12)
ATTENTION:
Online Quick Count for SY 2018-2019 have been deployed and opened. Schools will
automatically asked to enter Quick Count upon login.

 Public Schools - quick count data should be from June 4 - June 8, 2018.
 Private Schools/SUCs - quick count data should be from 1st week of opening of classes.

Deadline of Quick Count encoding - June 29, 2018.

For those schools who will not be able to finalize and submit their EOSY due to system
issues, finalization will be automated once the ICTS have already resolved schools' system
issues.
To all schools that have not yet finalized their EOSY due to pending issues, please accomplished

LIS (School Forms)

Pursuant to DepEd Order No. 58, s. 2017 (Adoption of New School Forms for Kindergarten,
Senior High School, Alternative Learning System, Health and Nutrition and Standardization of
Permanent Records), electronic copies of the following school forms are now available:

 Alternative Learning System (ALS) Forms (AF1 to AF5)

 School Form 5: Report on Promotion and Level of Proficiency for Kinder (SF5K)
 School Form 8: Learner's Basic Health and Nutrition Report (SF8)
 School Form 10: Learner's Permanent Academic Record for Elementary School (SF10)
 School Form 10: Learner's Permanent Academic Record for Junior High School (SF10)
 Senior High School Forms (SHSF-1 to SHSF-7, SF8-SHS)
 School Form 10: Senior High School Student Permanent Record (SF10-SHS)

It has come to our attention that so-called “electronic school forms” are being sold to public schools.
Please be reminded that DepEd has authorized NEITHER the usage of such “forms” NORtheir sale
to our schools.
School forms generated using this “solution” are considered NULL AND VOID, and will NOT be
considered for official transactions.
Official School Forms can be downloaded for FREE and are EXCLUSIVELY available for schools at
the Learner Information System (LIS).
SCHOOL FORMS ARE NOT FOR SALE.**
Report any incident of unauthorized use and sale of School Forms to
the LIS/EBEIS Helpdesk, the Planning Service, and the Office of the
Undersecretary for Planning and Field Operations.
