This is to inform you that you have been conditionally admitted to take the CAREER SERVICE
EXAMINATION, PAPER AND PENCIL TEST (Professional Level) to be held on April 17, 2016
(Sunday).

Details of your Room and School Assignment are, as follows:

• School Address : BRIXTON HILLS, BRGY. SANTOL, GALAS, QUEZON CITY

ATTENTION:
• The examination proper starts at 8:00AM. Please be at the examination venue at least 7:00 AM
for the preliminary activities. The gates of the school will be closed at exactly 7:30AM;
• Failure to appear on your scheduled date and time of examination will mean forfeiture of
examination fee and slot; and
• The time limit for the Professional Level is 3 hours and 10 minutes, while for the SubProfessional
Level is 2 hours and 40 minutes.

IMPORTANT THINGS TO BRING ON EXAMINATION DAY:

1. Recent valid ID Card with photo and signature. (The same ID Card presented at the time of
application.)
2. Application Receipt and/or Notice of School Assignment
3. Dark Lead Pencil, and
 4. Blue or Black Ballpen/s
