Instructions on completing an End User Statement

¾ Please list the product name, description (including the version/Rev number) and
Mitel part number if applicable.

(CONSIGNEE AT FINAL DESTINATION)

¾ For the purposes of the EUS, the consignee at final destination is the name of the
party that Mitel Networks is transacting business with. i.e. Mitel’s reseller / channel
partner name.

(Sufficiently describe the use such as: demonstration or evaluation of products,
protection of company proprietary and/or internal communications, secure
communication with clients, secure communication with branches, secure
communication with supply chain, provision of network/crypto service) at
(CONSIGNEE AT FINAL DESTINATION).

¾ Within this section, please explain what you do with the Mitel product(s) listed
above. If you are a reseller, explain that you resell Mitel products within your region.
Explain the types of business that you resell to (e.g.: hospitals, hotels etc). Also
include an explanation of what the Mitel product(s) are used for (e.g. Everyday
operations of incoming and outgoing telephone calls within a hotel).
