Requirements for Most Outstanding Student Leader
❏ Nomination Form
❏ One-page Curriculum Vitae
❏ Summary of the Nominee (up to 2,000 words for souvenir program, and a 100 word profile for media
purposes). Essay should be computerized font size 12, Arial, double spaced on 8.5”x11” bond paper, 1”
margins on all sides.
❏ Summary of Achievements (awards / recognitions, activities, projects, events). Do attach appropriate
proofs such as certificates, pictures, etc. Identify the levels of impact whether school-wide,
barangay/foundation, city-wide, regional, national, or international. No points for having no proofs or no
identified levels.
❏ One (1) recent, different, clear 2”x2” photograph labeled with the nominee’s full name and organization.

❏ Published speeches; list of awards and recognitions received; published articles like news articles,
website, blogs
